Before Your First Visit
Please remember to get any referrals you need for your first visit prior to arriving. For your convenience you can print some of our forms prior to your visit and bring them with you.
Please bring any radiographs, CT scans, MRIs on either film or disc and their reports that you have had prior to your visit. You may also want to bring any laboratory reports, operative reports or prior physician reports you feel are important. Remember to bring your insurance cards and a photo ID.

Please print, fill out and bring the completed intake forms with you to your appointment.
Instructions for completing fillable PDF forms:
- Install the most recent version of Adobe Reader (http://get.adobe.com/reader)
- Save the form to your local drive on your own computer and note the location of the saved file.
- Open the Adobe Acrobat Pro/Reader application.
- On the Acrobat Reader menu, go to File->Open. Open the file from the location in which you saved it.
- Complete the form and save the changes before emailing or printing.

We have early morning and evening appointments for your convenience. We will attempt to accommodate your urgent orthopedic needs, please call to schedule an available time. If you are unable to keep your scheduled appointment please contact us as soon as possible so we can use that time to see another patient in need of care. Please note our cancellation and No Show policies.
Fees & Payments
In an effort to decrease the cost of your medical care, we request payment be made for services at the time of your visit unless prior arrangements have been made. We accept cash, checks and most major credit cards. We will bill your insurance company if we are a participating provider. Please call if you have any questions concerning our billing or payment policies.
Prescriptions & Refills
For your safety, all prescriptions and refills are handled during our regular business hours when full access to your medical records are available.
